Notice of Intended Marriage (NOIM). Original Birth Certificates (if born in Australia). Current International Passport (if you are from another country). Change of Name Certificate. Decree Absolute (if you have been previously married and divorced). Death Certificate of spouse if you have been widowed. If any of the above documents are not in English, they will need to be officially translated by a legally recognized and authorized person. The Celebrant will need to see both the original documents and the translated documents. The Notice of Intended marriage Form (NOIM) (Form 13) may be downloaded from www.ag.gov.au/celebrants. The form must be completed and then signed by both Bride and Groom in the presence of one of the following: An Authorised Celebrant A Justice of The Peace A Commissioner for Statutory Declarations A Barrister or Solicitor A legally qualified Medical Practitioner A Member of the Federal Police A Member of a State or Territory Police Force The NOIM form needs to be received by me, no later than one month prior to the date of your intended marriage, and is only valid for 18 months. Also, if you are from overseas, the NOIM form needs to be signed and witnessed in the presence of one of the following officials, the official must also stamp and date the document.
